Student Forms

• Course Substitution Form

  Students must submit a 'Course Substitution Form' to the PhD Program Coordinator after reviewing the procedure in the PhD Handbook and obtaining approval from the Faculty of record "prior" to the start of the course start. Core courses cannot be substituted.

• Dissertation Defense Announcement

  All PhD Candidates should complete the 'Dissertation Defense Announcement' at least one (1) month, but no sooner than two (2) weeks of the Dissertation Defense date. This announcement must be provided to both the Program (for public posting) and the Graduate School.

• Dissertation Research Agreement

  To register for dissertation research, please download, complete and submit the Dissertation Research Agreement to the Ph.D. in Urban Studies Graduate Coordinator and the chair of your dissertation committee. Students cannot register themselves for dissertation research; department consent is required. Students with co-chairs are required to complete a different Dissertation Research Agreement (co-chairs).

• Examination or Thesis Only Agreement

  To register for this session, please download, complete and submit the Examination or Thesis Only Agreement to the relevant program coordinator and the chair of thesis, dissertation or final project committee. Students cannot register themselves for this course; department consent is required. Student with co-chairs are required to complete a different Examination or Thesis Only Agreement (co-chairs).

• Graduate Assistantship Application for Financial Aid

  To apply for an assistantship, please complete and e-mail the application, an electronic copy of your resume and an electronic copy of a writing sample to your program coordinator.

• Independent Study Agreement for Doctoral Students

  To register for independent study, please download, complete and submit the Independent Study Agreement for Doctoral Students to the Ph.D. in Urban Studies Graduate Coordinator, the professor who has agreed to work with you, and your advisor. Students cannot register themselves for an independent study session; they should contact their program coordinator after obtaining approval from the supervising faculty member. Please note that the program coordinator will not enroll students in an independent study course without written approval from the supervising faculty member (email notification is acceptable).

• Independent Study Agreement for Masters Students

  To register for an independent study, please download, complete and submit the Independent Study Agreement for Masters Students to the relevant program coordinator and the professor who has agreed to work with you. Students cannot register themselves for an independent study session; they should contact their program coordinator after obtaining approval from the supervising faculty member. Please note that the program coordinator will not enroll students in an independent study course without written approval from the supervising faculty member (email notification is acceptable).

  To register for an independent study, please download, complete and submit the Independent Study Agreement for Undergraduate Students to the undergraduate program coordinator and the professor who has agreed to work with you. Students cannot register themselves for an independent study session; they should contact their program coordinator after obtaining approval from the supervising faculty member. Please note that the Student Services Manager or program coordinator will not enroll students in an independent study course without written approval from the supervising faculty member (email notification is acceptable).

  This checklist shows the courses required to complete the PhD program.

• PLUS Personal Information Sheet (required for all Masters and Ph.D. applications)

  To apply for the Master of Science in Urban Studies program, the Master of Urban and Regional Planning program, or the Ph.D. in Urban Studies program, please download, complete and submit the Department of Planning and Urban Studies information sheet to the coordinator of the program to which you are applying.

• Thesis Research Agreement

  To register for thesis research, please download, complete and submit the Thesis Research Agreement to the relevant program coordinator and the chair of your thesis committee.
