There may be several forms which must be completed by students for varying purposes. There are distinctive procedures to follow. Understanding the procedures for the forms is very important. There is a reason for every step in the procedure and every line on the forms.
Sign Language or Real-Time Captioning for Courses/Campus Events
Students must submit course information or event information in detail to make a request for an interpreter or captioner. This can be done via email or using this form:

Without advance notice, we may not be able to secure services for the requested event. Requests sent outside of normal university hours will be addressed the following business day.
Students must notify the office if they drop a course or make course changes. A student must notify the office in advance of a meeting cancellation or absence from a class in order to meet our 48 hour cancellation requirement. We recognize that this may not always be possible in the case of illness or other circumstance beyond a student's control. No matter the circumstances or timeline before the event, please notify the office if you will not/cannot attend a class/event for which an interpreter or captioner has been requested.

Emotional Support Animal Guidelines/Documentation Requirements
In accord with the Fair Housing Act, UNO does provide reasonable and appropriate accommodations for a documented student with a disability for an emotional support animal in residence housing. The documentation submitted must be provided by a qualified evaluator. A qualified evaluator is a medical or mental health professional licensed in the state of the student's permanent residence or in the state of Louisiana. The documentation must be provided on or include the information on the Health Care Provider Documentation Request Form-ESA, that this office will provide you.
The review process may take up to or more than thirty (30) days. Students should submit their request at least 30 days prior to the date the student plans to move into a campus residence.
